imagine a tube- the ID being slightly larger than the OD of the shafts- with (3) equal spaced set screws on either end of the tube- slide shafts into either end, and tighten set screws- theres another way-
here is a web site of a manufacture that makes knucles and joints that mound directly to each shaft. I have use all kinds of these in the past they work well with miss alignment.
