There are several things that can cause this problem. One major issue is when the cut or feature "creates" a tangency with previous geometry in an area that isn't along the sketch plane. Try changing the dimensions of the shape you are extruding just slightly bigger and smaller and see where it will finally allow you to make the shape. Usually when you do that you begin to see where it has issues!
