Parameter Prompt - What am I doing wrong?

I've read through just about everything on here about prompting for parameter inputs, yet my form is still not working correctly. I'll detail what I've done. If you can find where I'm going astray, I'd really appreciate it.

My form is looking for simple things: title, filename, date, creator, etc.



Add in notes in my .frm file referencing all of the parameters:



Now when I create a new drawing using this form, I'm prompted for nothing.

Please help.

Well, I figured it out...

I DREW that table in the form editor. In order for custom parameters to work, they must be placed in an actual TABLE.

Wow. I'm elated and so frustrated at the same time.
